Warrant Request (2012-13)
This form is used for reimbursement after events and to release funds before an event. Please contact Kathy Leonard if you have any questions 314-9964 or email kathyishome@yahoo.com.

Deposit Verification Form (2010-11)
This form is to be used to deposit your funds from an event. Please put in PTA Financial Secretary file in the front office. If your deposit contains cash, make arrangements to deliver it personally to Heather Smith.

Copy Request Form (2010-11)
Please use this form to request copies be made for flyers and documents for PTA sponsored events and meetings.

Awards Nomination Form (2010)

Family Waiver and Family Waiver (Spanish)
This form is the "Parent's Approval and Student/Participant's Waiver" and is for parents to complete when dropping their kids off at PTA sponsored events (i.e. Movie Night, etc.).
